No problem!  Even though the HealthTrust-sponsored regional Biometric Health Screening events have come to a close for the 2018 program year, if you have HealthTrust medical coverage, you and your covered spouse can still have a screening at no cost to you and earn a $75 reward by choosing one of these options: 

• Have your screening at any ConvenientMD location in New Hampshire. Simply bring the Biometric Screening Authorization Form - ConvenientMD to any ConvenientMD location in New Hampshire any time from 8:00 a.m. to 8:00 p.m. seven days a week. You don't need an appointment -- just walk in! After your screening, your results will be sent automatically and securely to Onlife Health so you can receive your $75 reward.
• Have your screening at your own PCP’s office. Print the 2018 Biometric Health Screening Form – PCP. and complete the form with your primary care provider (PCP) at your next check-up. Then simply follow the directions on the form to submit it to receive your $75 reward. (Retirees with Medicomp Three coverage are not eligible to receive rewards for screenings with a PCP, but can qualify for a $75 reward by having a Biometric Health Screening at a ConvenientMD location.)

At your screening, you will learn the vital health numbers that give insight into your health.  These measurements include: blood pressure, blood sugar and cholesterol (blood sample taken by a simple finger prick, no fasting necessary); waist circumference; body weight and body mass index.To learn more about Biometric Health Screenings and all components of the Slice of Life program, log in to your secure HealthTrust account and click on the Slice of Life icon.

Please note: You must have your screening no later than November 30, 2018 to receive the 2018 Biometric Health Screening reward.
